The purpose of the Safety Award Program is to recognize employees who have demonstrated safety in an exceptional manner in the performance of their job duties with LPG.
All employees are eligible for nomination, excluding elected officials, appointed employees, and persons who have received the award within the past year. Nominees must have been employed by Lafourche Parish Government for a minimum of six months.
Using the following criteria, please explain why this employee should be considered for the Lafourche Parish Safety Award:
All nominations must be submitted by 4:30 pm on the last working day of the quarter. (Submittal deadlines: March 31, June 30, Sept. 30, Dec. 31) Nomination forms can be downloaded here to be submitted to the HR Director at the address below or submitted online using the order nomination form below.
